what is Spravato?
Spravato, also known by its generic name esketamine, is a prescription nasal spray used for the treatment of certain types of depression, specifically Treatment-Resistant Depression (TRD) and Major Depressive Disorder (MDD) with acute suicidal ideation or behavior. It is a form of ketamine, which has been modified slightly in its chemical structure to enhance its effectiveness and safety profile for treating depression.
Mechanism of Action
Spravato works differently from traditional antidepressants. It targets the NMDA (N-methyl-D-aspartate) receptor in the brain, which is part of the glutamate system. This mechanism is believed to help restore synaptic connections in brain regions involved in mood regulation, which can be impaired in patients with depression.

Spravato Usage and Indications
- Treatment-Resistant Depression (TRD): Spravato is used in conjunction with an oral antidepressant in adults who have not responded adequately to at least two different antidepressant treatments.
- Major Depressive Disorder (MDD) with Acute Suicidal Ideation or Behavior: It is also approved for the short-term treatment of depressive symptoms in adults with MDD who are experiencing acute suicidal thoughts or behaviors. However, it is not intended to prevent suicide or reduce suicidal ideation.

Safety and Monitoring
Due to its potential for abuse and the need for close monitoring, Spravato is only available through a restricted program under a Risk Evaluation and Mitigation Strategy (REMS). This program ensures that the medication is used safely and effectively, with patients being closely monitored during and after administration.
Efficacy
Clinical trials have shown that Spravato can be effective in rapidly reducing depressive symptoms in patients with TRD and in those experiencing acute suicidal ideation. However, it is typically used as part of a broader treatment plan that includes traditional antidepressants and psychotherapy.
